emergency forms for each child along with a list of children
riding in your vehicle.

2. Put nametags on children with the name of the center,
the center address and phone number.

3. If additional parents are going, separate the children so
that each parent and teacher is responsible for three or four
children.  Otherwise, have each child choose a partner to
be with during the entire trip.

which children are present on the day of the field trip.  In
addition, each teacher should know the number of children
attending the field trip.

5. Take roll by calling each child’s name and making visual
contact with him/her.

6. Take roll every time the vehicle is loaded and unloaded.

but also that every child has departed from the vehicle.
Write the number of children down on the firm provided.
Each parent/teacher group will receive emergency field trip
forms on each child in their group.

of the children they are responsible for.  Also the teacher
should check each vehicle before anyone departs from any
location to be sure each child is accounted for.

8. Take a final roll check when the children have returned to
the classroom.

9. Periodically during the field trip, each teacher should take
a head count of the children to be assured that the entire
group is still together.

10. Once every child is in the vehicle, the teacher should
buckle each child’s seatbelt.  For the older children who can
buckle their own seatbelt, the teacher should pull on the belt
to be sure it is secure.
